What we collect
When you book a consultation, submit the contact form, or email or call us, we receive the information you provide: name, organization, email, phone, and anything you write in the message. The booking and contact forms are provided by our CRM vendor (GoHighLevel/LeadConnector), which processes that information on our behalf. We do not ask for, and you should not submit, patient or protected health information through this website.
How we use it
To respond to you, schedule and hold the consultation you asked for, and follow up about our services. If you become a client, information about your facility is handled under the client agreement and, where applicable, a business associate agreement — not under this notice.
What we do not do
We do not sell personal information. We do not share it with third parties except the service providers who run our website, CRM, email and scheduling tools, and as required by law.
Cookies and analytics
This site loads a booking calendar and a contact form hosted by our CRM vendor (GoHighLevel / LeadConnector). Those embedded tools may set cookies needed to operate and may receive what you enter into them, under that vendor’s own privacy practices — see the vendor’s privacy policy for details. We do not run advertising or analytics trackers on this site; if we add any, this notice will be updated first.
Your choices
Email info@smartmbservices.com to ask what information we hold about you, to correct it, or to have it deleted. California residents may make these requests under the California Consumer Privacy Act where it applies; we will respond within the time the law allows and will not treat you differently for asking.
Do Not Track
We do not track visitors across third-party websites over time, and we do not respond to browser Do Not Track signals.
Changes
We will post any change to this notice on this page and update the effective date above.
